NPPES

NPPES is the National Plan and Provider Enumeration System. CMS uses it to assign National Provider Identifiers and to publish the public registry. The monthly file dated October 12, 2026 is the V2 dissemination file. A page on this site is a copy of the public registry, not the official one. The lookup is at /npi-lookup.

NPPES

The official registry is at npiregistry.cms.hhs.gov. This site does not show the mailing address from the registry. It shows the location address when the public record has one.
