Remittance advice

A remittance advice is the statement a payer sends to explain how it processed a claim. Medicare's electronic version is the ERA, and the paper version is the SPR. It lists the billed amount, the allowed amount, and the adjustment reasons. This site does not store remittances and does not list denial reason codes.

Adjustment reason codes are copyrighted code sets. They are not published on this site. The remittance itself is the source for a specific claim.

Does this site list CARC or RARC codes?
No. Those code sets are not published here. A remittance from the payer is the place to read them for a claim.
