Type 1 NPI

A Type 1 NPI is the National Provider Identifier for an individual person, such as a physician, nurse, or therapist. It stays with that person when they change jobs. A Type 2 NPI is for an organization. Both are 10 digits, and the tenth digit is a check digit. The lookup is at /npi-lookup.
