Medicare opt-out

A Medicare opt-out is an affidavit from a physician or practitioner who chooses not to bill Medicare for two years. CMS publishes the affidavits with the NPI, specialty, state, and effective dates. This site looks up one NPI at a time in that public file. Use the Medicare opt-out lookup.
