ADT notification

An ADT notification is an electronic alert a hospital sends when a patient is admitted, discharged, or transferred. Medicare rules at 42 CFR 482.24(d) require hospitals with qualifying electronic records systems to send them. Recipients include the patient's primary care practitioner or group and post-acute providers. Practices use them to start transitional care outreach.

What an ADT notification is

ADT stands for admission, discharge, and transfer. An ADT notification is the message a hospital system generates when one of those events happens. It usually travels as an HL7 ADT message, often routed through a health information exchange or a notification vendor. It carries at least the patient's name, the treating practitioner, and the sending hospital.

Since 2021, the Medicare Conditions of Participation at 42 CFR 482.24(d) have required it. A hospital whose electronic records system meets the federal content exchange standard must send these notifications. They cover inpatient admission, discharge, and transfer, plus emergency department registration, discharge, or transfer. Recipients include post-acute providers and the primary care practitioner or group the patient identifies.

A discharge alert starts the clock on transitional care management. TCM requires interactive contact with the patient within 2 business days of discharge. Are hospitals required to send ADT notifications?

How do practices get ADT feeds?
Most practices get them through a state or regional health information exchange, an ACO or payer feed, or a commercial notification vendor. The practice registers its patient panel. The service then matches hospital events to those patients and sends alerts by portal, email digest, or file.
