CAHPS

CAHPS (Consumer Assessment of Healthcare Providers and Systems) is a family of standardized surveys that ask patients about their experience of care. The Agency for Healthcare Research and Quality oversees it. Medicare uses CAHPS results in Medicare Advantage and Part D Star Ratings, hospital programs, and ACO quality scoring, so experience scores affect payment.

What CAHPS is

CAHPS stands for Consumer Assessment of Healthcare Providers and Systems. The Agency for Healthcare Research and Quality (AHRQ) oversees the program. Each survey asks patients to report what happened during their care, not just whether they were satisfied. Typical topics include getting timely appointments, how well doctors communicated, and getting needed care and prescriptions.

There are versions for different settings. HCAHPS covers hospital stays, the Clinician and Group survey covers practices, and MA and PDP CAHPS covers Medicare health and drug plans. Starting with the 2026 Star Ratings, CMS cut the weight of patient experience, complaints, and access measures from 4 to 2.

Experience scores are hard to move with a survey alone, because they reflect what patients remember. How much do CAHPS measures count in Star Ratings?
Starting with the 2026 Star Ratings, CMS weights patient experience, complaints, and access measures at 2, down from 4. Outcome and intermediate-outcome measures carry a weight of 3. CAHPS still matters, but it no longer outweighs clinical and adherence measures.
Is CAHPS the same as a patient satisfaction survey?
No. Satisfaction surveys ask how happy a patient was. CAHPS asks patients to report specific experiences, such as how often they got an appointment as soon as they needed one. That makes results comparable across organizations, which is why CMS uses CAHPS for public reporting and payment.
Which CAHPS measures is CMS removing from Star Ratings?
The CY2027 Medicare Advantage final rule removes several measures starting with the 2029 Star Ratings. CAHPS-based measures on that list include Customer Service and Rating of Health Care Quality. Other CAHPS measures stay in the program, at the lower weight of 2.
