Medication Therapy Management (MTM)

Medication Therapy Management is a set of pharmacist-led services that make sure a patient's medications are working safely and as intended. The core service is the Comprehensive Medication Review: a one-on-one review of everything the patient takes, ending with a written medication list and action plan.

Part D plans must target members with multiple chronic conditions, multiple covered drugs, and drug costs above a threshold. Plans can require no more than three chronic diseases and no more than eight Part D drugs, and the 2026 cost threshold is $1,276. The CMR completion rate is display-only in the 2027 Star Ratings and returns as a new measure in 2029.

The hard part is contact: eligible members have to be reached, scheduled, and followed up. Rivvi runs that outreach as conversations, inviting members to their review, handing interested members to pharmacy staff by warm transfer or task, and honoring opt-out on every attempt.
