Is Google Voice HIPAA compliant?

Only on a plan where the vendor signs a BAA. Google Voice is not HIPAA compliant on its own. Google Voice in Google Workspace (managed users only): Google lists Google Voice in the BAA for managed users only, and says an administrator must accept the BAA before the organization uses PHI.

Yes, with a limit. Google lists Google Voice among the Google Workspace services included in the Google Cloud BAA, for managed users only.

Is a personal Google Voice number covered?

Google's list covers Google Voice for managed users only. Google also says customers who have not signed a BAA with Google must not use PHI in Google Workspace.

What has to happen before a team uses PHI in Google Voice?

Google says administrators must review and accept a BAA before using PHI in Google services. A super administrator can see the accepted BAA in the Admin console under Account settings, Legal and compliance.
