Why it matters
No BAA means no PHI, free or not
HIPAA requires a signed Business Associate Agreement between a covered entity and any vendor that handles protected health information. It is not optional and not a formality: without it, running patient data through an AI tool is a compliance violation.
Vendors know this, which is why the BAA is the most common thing gated behind a paid or enterprise plan. A free plan without a BAA can demo on fake data, but it cannot legally make a single real patient call.
