C-SNP

A C-SNP is a Chronic Condition Special Needs Plan. It is a Medicare Advantage plan that limits enrollment to people with a severe or disabling chronic condition that CMS allows for that plan. It is not a D-SNP and not an I-SNP. Star Ratings still apply to the contract. The ratings lookup is at /star-ratings-2027.

C-SNP

The condition list is set by CMS for the plan. This page does not confirm that a person qualifies.
