A CMS-HCC is a Hierarchical Condition Category in the Medicare Advantage risk model. CMS maps ICD-10-CM codes to category numbers. This site uses the 2027 initial mapping file. Codes added for FY2027 that have no row say that no mapping is published yet. The lookup is at /icd-10.
The page label is CMS-HCC, payment year 2027 (initial mapping). It prints the category number. It does not print a model version name. It does not compute a risk score.
