Definition
A deactivated NPI is a number NPPES has turned off. The file records a deactivation date and a reason code. A later reactivation date means it was turned back on. This site treats a record as active when there is no deactivation date, or when reactivation is on or after deactivation. The lookup is at /npi-lookup.
CMS also publishes a separate deactivated NPI report. The October 12, 2026 report is the V2 file dated that day. A deactivation is not an OIG exclusion.
