PDC and fail date calculator

Enter a patient's fill dates and days' supply for one medication. The calculator returns their Proportion of Days Covered (PDC) so far, their projected PDC if they never refill, and their fail date: the last day they can refill and still finish the measurement year at the 80% adherence threshold Medicare Part D Star Ratings use.

Fills

How it's calculated

PDC is the share of days in the treatment period a patient had the medication on hand. The treatment period starts at the first fill and ends at the end of the measurement year. Overlapping supply from an early refill is shifted forward, so a day is never counted twice. The fail date is the last refill date that leaves the patient at or above the threshold by year end, assuming they stay covered after refilling. The Pharmacy Quality Alliance defines the measure; Medicare Part D Star Ratings use it for statins, RAS antagonists, and diabetes medications.

Questions

What PDC counts as adherent?
80% for most chronic medications. PQA uses 90% for antiretrovirals. Medicare Part D Star Ratings count the share of members at or above 80% for statins, RAS antagonists, and diabetes medications.
Why does the calculator say "not yet measurable"?
PQA's measure requires at least two fills on different dates, and the first fill must come at least 91 days before the measurement period ends. Until then the patient isn't in the measure.
Why does an early refill not raise PDC immediately?
Early-refill supply is pushed to start after the current supply ends, so it counts toward future days, not days the patient already had covered.
How far ahead of the fail date should we call?
A common practice is to start outreach four to six weeks ahead, which leaves time for pharmacy processing, callbacks, and barriers like cost or transportation. That is practice guidance, not a CMS rule.
