NPI reactivation
NPI reactivation is the date NPPES turns a deactivated number back on. The October 12, 2026 dissemination file has a deactivation date and a reactivation date. This site treats the record as active when the reactivation date is on or after the deactivation date. A reactivation is not a new NPI. The lookup is at /npi-lookup.

Does reactivation issue a new number?
No. The same NPI is turned back on. A new enumeration would be a different number.
