Is Typeform HIPAA compliant?

Enterprise: Typeform says the client must notify Typeform so it can assess offering HIPAA compliant services and sign a BAA attached to the enterprise agreement. Its pricing page lists HIPAA compliance as Enterprise only.

Will Typeform sign a BAA?
Possibly, on Enterprise. Typeform's usage policy says a client that intends to collect HIPAA information must notify Typeform, which will assess offering HIPAA compliant services and signing a Business Associate Agreement.
Can we collect PHI in Typeform before a BAA is signed?
No. Typeform says that unless a Business Associate Agreement is signed, the client shall not submit information subject to HIPAA requirements.
Which Typeform plans offer HIPAA compliance?
